Ophthalmic Compounding, Built for the OR and the Clinic

For over 30 years, Greenpark Compounding Pharmacy has been the partner Houston ophthalmologists and optometrists rely on for preservative-free, sterile, and non-commercially available ophthalmic formulations. From fortified anti-infectives prepared under USP <797> standards to low-dose atropine for pediatric myopia, our team formulates to your protocol — not a manufacturer’s SKU list.

Preservative-Free Eye Drops, Formulated for Sensitive Eyes

BAK and other preservatives drive ocular surface toxicity, disrupt tear film stability, and limit how often patients can dose.
For patients with dry eye disease, post-surgical needs, glaucoma on multi-drop regimens, or documented preservative sensitivities,
preservative-free formulations are often the clinical requirement — not a preference.

What We Compound PF:

  • Fortified antibiotics
  • Antifungals
  • Steroids
  • Cyclosporine
  • Tacrolimus ophthalmic
  • Custom anesthetic drops for in-office procedures

Delivery Systems Available:

Single-use vials, multi-dose PF bottles with tip-seal technology, and unit-dose droppers.
We match the delivery system to the protocol — not the other way around.

Fortified Anti-Infectives for Aggressive Ocular Infections

When microbial keratitis, endophthalmitis, or Acanthamoeba are present, commercially available concentrations aren’t enough. Greenpark compounds the fortified and specialty anti-infectives ophthalmologists reach for when the standard bottle won’t cut it.

Antibacterial (Fortified):

  • Vancomycin

  • Tobramycin, fortified concentrations

  • Cefazolin

  • Amikacin, Gentamicin, Ceftazidime — fortified

  • Moxifloxacin, Gatifloxacin in custom concentrations

  • Polymyxin B / trimethoprim combinations

Antifungal:

  • Voriconazole

  • Amphotericin B

  • Natamycin

  • Fluconazole ophthalmic

Antiviral:

  • Ganciclovir

  • Acyclovir ophthalmic formulations

Acanthamoeba Keratitis Protocol Compounds:

  • PHMB (Polyhexamethylene biguanide)

  • Chlorhexidine

  • Propamidine isethionate

  • Pentamidine Isothionate

Most anti-infective orders will be ready on the same day. For sight-threatening cases where timing matters, call us directly — we triage same-day preparation whenever clinically feasible.

Beyond Artificial Tears — Targeted Therapy for Complex Dry Eye

Commercial dry eye therapy is limited to a handful of branded options at fixed concentrations. For patients who fail first-line treatment, have concurrent ocular surface disease, or need higher-potency or preservative-free alternatives, compounding opens the full therapeutic range.

Dry Eye Compounds We Formulate:

  • Spironolactone ophthalmic drops patented

  • Dapsone ophthalmic drops

  • Losartan ophthalmic drops

  • Cyclosporine

  • Tacrolimus ophthalmic

  • N-Acetylcysteine (NAC) drops for filamentary keratitis

  • Albumin eye drops

  • Heparin drops in custom concentrations

  • Vitamin A (retinyl palmitate) ophthalmic ointment

  • Testosterone ophthalmic drops and periocular cream

  • Autologous serum tears

  • Platelet-rich plasma (PRP) drops

Serum Tears Workflow:

We coordinate directly with your patient’s blood draw (in-office or contracted lab), process under sterile conditions, and ship to the patient or your clinic. Patients receive detailed handling and storage instructions. Turnaround is typically 3–5 business days from specimen receipt.

Custom Steroid Strengths for Post-Surgical, Inflammatory, and Uveitis Protocols

Standard prednisolone 1% doesn’t fit every protocol. Whether you’re managing post-LASIK, cataract recovery, uveitis flares, GvHD of the eye, or high-risk corneal grafts, we formulate the steroid, concentration, vehicle, and preservative status your patient needs.

Available Steroid Compounds:

  • Prednisolone acetate — preserved or PF

  • Prednisolone sodium phosphate PF

  • Dexamethasone— drops, ointment, PF options

  • Methylprednisolone ophthalmic

  • Loteprednol custom concentrations (combination with Ivermectin)

  • Triamcinolone for injection (preservative-free, for intravitreal/subconjunctival use at prescriber discretion)

  • Steroid + antibiotic combinations to protocol

All steroids prepared sterile under USP <797> in our ISO-classified cleanroom.

Low-Dose Atropine — Precisely the Strength You Prescribe

Pediatric myopia progression is one of the most common reasons clinics come to Greenpark. Commercial atropine is 1% — far too concentrated for myopia control protocols. We compound the low-dose strengths studied in LAMP, ATOM, and CHAMP protocols, in preservative-free formulations appropriate for pediatric nightly dosing.

Atropine Strengths We Compound:

  • 01%

  • 025%

  • 05%

  • 1%

  • Custom strengths on request (up to 1%)

Formulation Options:

  • Preservative-free unit-dose vials

  • Multi-dose PF bottles

  • Preserved multi-dose (when clinically appropriate)

  • Buffered or unbuffered, per your protocol

For Pediatric Practices:

We coordinate refill reminders directly with parents, offer 3-month supplies to reduce pharmacy trips, and ship directly to the home. Most myopia control prescriptions ship within 2 business days.

We also compound higher-concentration atropine for amblyopia penalization, cycloplegic refraction, and pre-operative use.

Ocular Surface Neoplasia & Specialty Oncology Formulations

For ophthalmologists managing ocular surface squamous neoplasia (OSSN), conjunctival lymphoma, and other surface malignancies, we prepare the topical chemotherapeutic agents your protocol requires — under strict USP <800> hazardous drug handling standards.

Anti-Neoplastic Ophthalmic Compounds:

  • Mitomycin-C (MMC) — 0.02%, 0.04%, 0.05% custom concentrations

  • 5-Fluorouracil (5-FU) — 1%, custom

  • Methotrexate ophthalmic

Compliance & Handling:

All hazardous drug compounding is performed in our dedicated USP <800> negative-pressure cleanroom by technicians trained specifically in hazardous drug handling. Shipping and patient-handling instructions accompany every order.

These are specialty preparations — we recommend calling ahead to confirm lead time and coordinate any patient-specific requirements.

Targeted Therapy for Demodex Blepharitis

Demodex is present in up to 70% of chronic blepharitis cases, and commercial options are limited and expensive. Greenpark compounds the topical and lid-margin therapies ophthalmologists and optometrists have used successfully for years, at strengths and in vehicles tailored to patient tolerance.

Demodex & Ophthalmic Parasitic Compounds:

  • Ivermectin 1% lid cream / ointment

  • Metronidazole ophthalmic ointment (0.75%, 1%, 2%)

  • Tea tree oil (terpinen-4-ol) lid scrubs in controlled concentrations

  • Combination lid therapies (ivermectin + metronidazole)

Why Compounded Demodex Therapy:

Patient tolerance varies widely. We titrate concentration and vehicle (ointment vs. cream vs. foam) to the patient, reducing burning and irritation that causes discontinuation on commercial products. Pediatric-safe and pregnancy-considerate formulations available on request.
